The Nebraska Chronic Disease Prevention & Control Program, within the Division of Public Health, is seeking highly motivated, self-directed health professionals to serve as the Diabetes Management and Education Coordinator and Chronic Disease Prevention & Control Program Evaluator. These individuals will provide innovation, leadership, and guidance to organizations and partners in Nebraska to grow and strengthen the chronic disease prevention and management initiatives in the state. These positions work as part of a collaborative team to assure effective implementation and evaluation of program activities.
Diabetes Management & Education Coordinator
This position will serve as the subject matter expert for diabetes management and education programming with a focus on improving access to and participation in Diabetes Self-Management Education & Support (DSMES) programs; enhancing DSMES coverage among insurers or employers; and improving quality of diabetes care among healthcare providers.

Chronic Disease Prevention & Control Program Evaluator
