The Nebraska Chronic Disease Prevention and Control Program, within the Nebraska Department of Health & Human Services, currently has two open positions. We are seeking motivated and self-driven professionals to provide innovation, leadership, and guidance to organizations and partners in Nebraska.

Chronic Disease Self-Management Programs Coordinator – Closes 7/6/2020 11:59 PM CST

This position will predominately serve as the Evidence-Based Programs Coordinator (75%) and will assist with pharmacy-based initiatives aimed at improving management of diabetes, hypertension, and cholesterol (25%). Evidence-based programs currently include the Living Well Programs (Chronic Disease Self-Management Program, Diabetes Self-Management Program, Chronic Pain Self-Management Program) and Healthy Heart Ambassador Blood Pressure Self-Monitoring Program. This position works as part of a collaborative team to assure effective implementation and evaluation of program activities.

Diabetes and Nutrition Programs Coordinator – Closes 7/10/20 11:59 PM CST

Serve as the subject matter expert for diabetes management and education programming with a focus on improving access to and participation Diabetes Self-Management Education & Support (DSMES) programs; enhancing DSMES coverage among insurers or employers; and improving quality of diabetes care among healthcare providers. This position works as part of a collaborative team to assure effective implementation and evaluation of program activities.
