We are poised in a new era of technology and big data. This presentation will review sophisticated efforts that build on increased availability of big data and improved analytics, and will describe how these efforts support integration of personal, practice, system, and community programs to improve individual and population health. Objectives: 1) Describe current and new tools and methods for understanding big data 2) Describe how these tools and methods are being utilized to integrate health and healthcare at the individual, practice, system, and community level; and, 3) Discuss the potential impact of using these tools and methods, with local data, to demonstrably improvement in health.
