Participant and Team Captain Logging Forms

Download these forms to log your physical activity for the 6 weeks of the Fitness That Works Physical Activity Program. Recording begins on Monday, April 6th and concludes on Sunday, May 17th.

For team members to earn their stretching straps every team member must log one or more activity 5 or more days each week of the program. At the conclusion of each week, the team captain will provide the average points per participant for their team and report whether the team is still qualified to earn their stretching straps.
