Work schedule: Mon-Fri 8 am to 5 pm
Examples of Work
Working with health systems or clinics serving disparate populations to assist them in implementing evidence based practices to increase cancer screening rates, and to increase hypertension control. This position is responsible for several key activities that integrate core components across funding sources: Health Systems Change, community clinical linkages, bi-directional referrals between community and primary care, and overseeing sub recipients receiving integrated funding awards. Will supervise staff working on core activities around community clinical linkages, and public health campaigns. Another essential responsibility will be collaborating with partners and bringing partners together to develop preventive screening pathways to care. |
